It's been largely the combination of lack of resources , both medical and educational, as well as the fact that some government and church institutions initially were putting out false information which was worse than no information.Poverty will mean that some entire countries won't be able to support HIV infected people, house or hospitalize them, or often even communicate education. Even when education can get spread, it may be at odds with local or national preconceptions or attitudes. Africa is many countries and many cultures, some tribal, some European and some in between, some of which have higher educational and economic resources than others; lumping all of Africa together isn't a fair representation of the problem.

HIV is a large problem in the african american community, because they still think it's a gay disease and it can't happen to them. Also, there are a lot of down low brothers. There is a stigma about being gay and black. So it goes on educated and the lack of awareness that there are gay blacks. This is made true in black music and media that it is not ok to be black and gay. That is why HIV infection in the african american community is so abundant.

I believe the number one cause is infidelity. Having mistresses is a status symbol. Often the girls are very young, so bleeding is a problem. Also there is a preference in some areas for "dry" sex, which will definitely ccause bleeding and cuts. Poverty, lack of good medical care, and the stigma of having it all add to the problem.

One issue: Lack of education about how HIV is actually transmitted/faulty beliefs: People in Africa believe that if they have HIV, and have sex with a virgin, they will be cured. This belief results in a high rate of rape of young people and rapid spread of HIV. Also, because access to medications is limited, women in Africa transmit the virus through pregnancy, delivery, and breastfeeding more often than women in the US do. (In the US, women can take meds to reduce the risk of transmission of the virus during pregnancy, they can have C-sections, and avoid breastfeeding, to further reduce risk of HIV transmission to the baby.)
